That third condition is the one people forget, and it's where most attempted fortresses collapse. You build a beautiful wall and then discover you're in zugzwang, because every legal move you have breaks something. A fortress with no safe waiting move is just a slow loss.

Why engines get these wrong
This is the genuinely interesting part, and it matters if you analyse with an engine.
An engine evaluates by searching ahead and scoring positions. A fortress is a claim about *infinity*: that no sequence of moves, ever, breaks through. You can't verify that by searching twenty or thirty moves deep, because the attacker can shuffle around for fifty moves and the position looks the same.
So the engine sees a rook for a bishop, adds up material, and reports a large advantage. It's wrong, and it stays wrong no matter how long you let it think, until the fifty move rule starts appearing in its search horizon.
Neural network engines handle this better than the old handcrafted evaluation ones did, because they learned position patterns rather than counting material, and some fortress structures are recognisable patterns. But it's still a known weak spot, and you should not treat a big evaluation number as proof in a closed position.
The practical rule I use: if an engine says plus three but the position is completely blocked and there's no obvious pawn break, be suspicious of the number. Look for the break yourself. If you can't find one, the engine probably can't either, it's just not admitting it.

Building one before it's too late
The hardest part is that fortresses have to be constructed while you still have choices, and by the time you're desperate you usually don't.
That means recognising early, while you're merely worse rather than lost, that the position is heading somewhere you can't hold by normal means. Then you deliberately steer toward a structure that's drawable rather than one that's objectively less bad.
That's a genuinely counterintuitive decision. You're choosing a position an engine rates worse over one it rates better, because you understand something it doesn't about what's convertible.
Concrete habits that help:
Fix your pawns on squares the enemy bishop cannot attack. If they have a light squared bishop, put your pawns on light squares where the bishop can't touch them.
Trade the pieces that can break in and keep the ones that defend. Rooks are usually the invaders, so trading a pair of rooks often helps the defender even when material stays lopsided.
Look for a blockade square in front of a passed pawn, ideally for a knight, which is the best blockader because it defends squares around itself while sitting still.
Do not panic and grab material back. A fortress usually requires you to accept being down material permanently, and the instinct to restore the balance will destroy the structure.
I've held exactly one properly constructed fortress in a rated game and it was the most satisfying draw of my life. Eventually you'll be the one facing a fortress, and the answer is usually one of three things.
A pawn break somewhere on the board that opens a second front. Almost all fortresses hold against pressure in one area and fail against two. Getting a passed pawn on the opposite wing forces the defender to stretch, and a defence that has to cover two places usually cracks.
A sacrifice to demolish the structure. Giving back material to remove the key defensive pawn is often the only way in. Counting material here is exactly backwards, since you have material to spare and what you need is access.
Zugzwang. If you can reach a position where the defender has no safe waiting move, the wall falls on its own. This is why that third fortress condition matters so much from both directions.
If none of those exist, take the draw and stop grinding. I've watched players spend forty moves in a drawn fortress position out of pure stubbornness, get low on clock, and lose. Being right about the evaluation is worth nothing if you flag.
